Degeneration on a scan is neither necessary nor sufficient for back pain: disc degeneration and bulges are extremely common in people with no symptoms, and whether the nervous system is turning pain up or down does not show on imaging. So major guidelines do not recommend routine imaging for ordinary back pain without red-flag symptoms; first-line care is a clear explanation and staying active.
Central sensitization means the pain-processing circuits become more sensitive, so the same input is amplified into more pain. The shift of tension-type headache from occasional attacks to near-daily chronic pain is thought to involve it; episodic and chronic forms may rest on different mechanisms (Ashina 2021).
Wear and disc bulges on a neck scan are common in people who never have neck pain, and whether the nervous system is turning pain up or down does not show on a scan. So scan findings often do not match neck pain, and for ordinary neck pain with no red flags and no nerve symptoms, routine imaging is not the first step.
