抽烟 · 烟在身体里做了什么
一口烟同时送进氧化物、一氧化碳和尼古丁,分别让维生素 C 消耗更快、占住运氧的位置、压住体重。这里讲戒烟后身体按什么顺序恢复,以及吃上该注意什么。

一口烟里吸进了什么
What one puff carries in
抽烟伤身,不是靠某一种毒物,而是一口烟同时把几类东西送进身体,它们各走各的路。
烟分两部分:飘着的气体,和凝成细小颗粒的焦油。光是颗粒那一部分,就有至少 3500 种化合物,很多有毒、能致癌或让基因突变。气体里有一氧化碳,还有大量自由基(少了一个电子、会去抢别的分子电子的活泼分子),估计每吸一口约有 10¹⁵ 个,也就是一千万亿个(Valavanidis 综述)。
进到肺里以后,它们分头行动:

机制 · 焦油里的自由基为什么不散
气体里的自由基寿命很短,但它们处在一种稳态里:不断被消耗,又不断新生成,烟放得越久,浓度反而越高(Valavanidis 综述)。焦油里的不一样。每克焦油带着约 10¹⁷ 个稳定的自由基,它们来自一套醌类化合物,能反复把电子交给氧气,先生成超氧阴离子,再生成过氧化氢和最活泼的羟自由基(Pryor 综述)。这套系统用掉一轮还能再转一轮,所以焦油沉在气道里,就成了一个持续产生氧化物的小作坊。
实验室里还看到几件事:
焦油的提取液能让 DNA 生成一种氧化损伤的标记物(8-OHdG)焦油里的自由基能钻进活细胞、结合到 DNA 上、把 DNA 链切出缺口普通滤嘴并没有明显减少烟里的自由基

一氧化碳占住运氧的位置
Carbon monoxide takes oxygen's seat
红细胞里的血红蛋白负责在肺里装上氧气,再送到全身。一氧化碳能结合在同一个位置上,而血红蛋白对它的亲和力是对氧气的约 250 倍(Rose 综述)。抽烟吸进来的一氧化碳,就这样占住了一部分运氧的座位。
麻烦还有第二层:一氧化碳结合上去以后,会让同一个血红蛋白上剩下的位置把氧抓得更紧,到了组织里更不肯放手。于是血里装的氧变少,交给组织的氧也变少。它还能结合心肌和骨骼肌里储氧的肌红蛋白。
医生看血里被一氧化碳占着的血红蛋白比例时,对两类人用不同的线:不抽烟的人 2% 以上算异常,抽烟的人要到 10% 以上。两条线不同,本身就说明抽烟的人平时血里带着更多一氧化碳。
好在它走得也快:只呼吸普通空气时,一氧化碳的半衰期约 320 分钟。

机制 · 停抽后一氧化碳多久排掉
半衰期是指剩下的量减半所用的时间。约 320 分钟减一半,按这个速度算,过 5 个半衰期,也就是约 27 小时,多出来的一氧化碳只剩约三十分之一。所以停抽后一天多,血里这一部分基本回到不抽烟的水平。要读准这个数:320 分钟这个半衰期来自一氧化碳中毒的研究,上面那一天多是按它算出来的,不是在戒烟的人身上逐小时测出来的时间表。

抽烟时的一氧化碳浓度比中毒低得多,这两点说明的是它能到哪里、会做什么,不是说抽烟等于中毒。

烟让维生素 C 用得更快

维生素 C 是血液和组织里最先出手的抗氧化剂之一:氧化物来了,它把电子让出去,自己就被用掉了。烟里源源不断的氧化物,让这笔消耗一直开着。
一项用同位素标记维生素 C 的小型研究(Kallner 1981),在每天抽 20 支以上的男性身上测到:他们体内维生素 C 的周转比不抽烟的人更快。吃进去同样多,抽烟的人留下的更少。
美国医学研究院(IOM)的膳食参考摄入量据此给吸烟者在推荐量之外每天多加 35 mg,理由是吸烟增加了氧化压力和维生素 C 的代谢周转。
这点差额从食物里就能补上,新鲜蔬菜和水果是主要来源,维生素 C 那一篇列了常见食物的含量。要记住它补的只是一笔账:按机制推,多吃维生素 C 能把被烟多用掉的那份补平,挡不住烟里的其他东西。

证据 · 吸二手烟的人也会少吗
会,至少在一项研究里是这样。Tribble 1993 在美国比较了三组人:44 名经常吸二手烟的不吸烟者、47 名吸烟者和 50 名没有烟草暴露的人。吸二手烟的人,血里的维生素 C 正好落在两组中间:比吸烟者高,比没暴露的人低,而三组从饮食里吃进的维生素 C 差不多血里维生素 C 偏低(低于 23 μmol/L)的,吸烟者里有 24%,吸二手烟的人里有 12%,没暴露的人里一个都没有只有在受烟暴露的两组里,吃得少的人血里才明显偏低
作者的结论是:长期受烟暴露,尤其再加上维生素 C 吃得少,可能让主动和被动吸烟者的维生素 C 储备都下降。
要读准它:这是一项小型横断面研究,只能说明这几组人在一个时间点上的差别。IOM 没有给吸二手烟的人单独定一个量。对家里有人抽烟的人,能拿走的是两件事:每天把蔬菜水果吃够;更根本的是别让烟进到室内。

β-胡萝卜素补剂为什么伤吸烟者

观察研究里,蔬菜水果吃得多、血里 β-胡萝卜素高的人,肺癌更少。于是研究者给吸烟者吃高剂量的 β-胡萝卜素补剂,想看能不能降低肺癌。结果是反的:
芬兰的 ATBC 试验,29133 名男性吸烟者,补 β-胡萝卜素的人肺癌多了 18%,总死亡高了 8%美国的 CARET 试验,18314 名吸烟者、戒烟者和石棉工人,β-胡萝卜素加维生素 A 让肺癌的升到 1.28,试验因此提前 21 个月叫停
美国预防服务工作组(USPSTF)据此建议,不要用 β-胡萝卜素补剂来预防心血管病或癌症。

机制 · 烟加大剂量为什么会出事
人身上的试验只回答了会不会出事,没回答为什么。一项雪貂实验(Wang 1999)给出了一种可能的解释。研究者把雪貂分成几组,6 个月里分别给高剂量 β-胡萝卜素、让它们吸烟,或两样都来:
所有补了 β-胡萝卜素的雪貂,肺组织里的细胞都在明显增殖,还出现了鳞状化生(气道表面换成了一种更硬、更扁的细胞),吸烟让这种变化更重三个处理组肺里的视黄酸(维生素 A 在细胞里起作用的形式)都偏低,一种视黄酸受体(RARβ)的表达少了 18% 到 73%补剂加吸烟的那组,两个促进细胞增殖的基因(c-Jun 和 c-Fos)表达高了 3 到 4 倍
作者据此提出:维生素 A 这条管细胞正常分化的信号被削弱,可能是肺里更容易长出肿瘤的一种机制。
要读准它:这是动物实验,在人身上没有被直接证实。另一种常被提起的解释,是大剂量 β-胡萝卜素在烟和高氧的肺里被氧化后反过来产生自由基,这一步同样没有在人体里测过。

戒烟后为什么容易长胖
Why weight goes up after quitting
尼古丁从两头压住体重:它让身体一天多烧约 10% 的能量,又通过大脑里管食欲的信号让人吃得少(Audrain-McGovern 综述)。所以平均来说,成年吸烟者比不抽烟的人轻 4 到 5 公斤。
一戒烟,两头同时松开:代谢率回落,饭量上来。一项研究里,戒烟的人平均每天多吃约 227 千卡,能解释戒烟 3 个月时约 69% 的体重增加。
汇总 62 项研究的一项 (Aubin 2012)给出了具体的形状:没用药物帮忙戒烟的人,12 个月时平均重了约 4.67 kg,大部分发生在头 3 个月;但人和人差别很大,约 16% 的人反而瘦了,约 13% 的人重了 10 kg 以上。
体重增加不该成为接着抽的理由:在美国 Framingham 队列里,戒烟后体重上升的人,心血管病风险照样明显下降(Clair 2013,观察性研究)。

实操 · 头三个月怎么接住
知道了机制,做法就推得出来:体重主要长在头 3 个月,而且主要来自吃得多,那就在这段时间提前安排好吃什么。把饭量的空档提前填好:按时吃饭,想吃零食时手边先有蔬菜、水果这类体积大、热量低的东西;这是按机制推的做法,不是试验验证过的方案动起来:代谢率回落的那一部分,活动量能补回一些;体重管理 · 根本逻辑讲了能量收支怎么算别指望药把体重也管住:综述指出,帮助戒烟的药物大多只是把体重增加推迟,停药以后体重照样会涨;里,用不同药物戒烟的人,12 个月时重的也差不多
关于心脏,Clair 2013 的数字是:没有糖尿病的人戒烟后 4 年内体重中位数增加 2.7 kg,和继续吸烟的人比,近期戒烟者心血管病风险的比值是 0.47,把体重变化也算进去以后几乎没变。有糖尿病的人方向一样,只是没有达到统计学意义。
有一个反直觉的现象也值得知道:一些研究里,抽得最多的人反而比抽得少的人更重。尼古丁压体重的作用,挡不住其他一起出现的生活习惯。

戒烟后身体按什么顺序恢复
What recovers after quitting, and when
戒烟的好处不是一次到位,而是身体各部分按自己的速度回来:
一氧化碳:半衰期约 320 分钟,按这个速度算,停抽后一天多,多出来的部分基本排掉了伤口愈合:组织里的供氧在停抽后很快恢复,负责清理和防感染的炎症细胞约 4 周内部分恢复,负责修补的成纤维细胞慢一些(Sørensen 综述)肺功能:一项大型戒烟试验里,有轻中度气流受限的吸烟者真正戒掉后,第一年肺功能平均回升 47 mL,之后每年下降的速度只有继续吸烟者的一半,和从不吸烟的人相当(Lung Health Study)心血管:在美国 Framingham 研究的重度吸烟者里,戒烟 5 年内,心血管病发生率已明显低于继续吸烟的人;要到 10 到 15 年,才和从不吸烟的人分不出差别(观察性研究)

安全 · 要做手术的人早点戒
手术伤口最能看出烟对修复的影响。一项汇总 140 项队列研究、47 万多名病人的 (Sørensen 2012)里,和不吸烟的人比,吸烟者术后:伤口组织坏死的约 3.60伤口愈合延迟或裂开约 2.07手术部位感染约 1.79骨头或瘘管长不好约 2.44
戒过烟的人比从不吸烟的人仍然多一些愈合问题(比值比 1.30),但比还在吸的人少(0.69)。4 项随机试验里,在手术前后帮病人戒烟,手术部位感染明显减少(比值比 0.43),其他愈合问题没有明显减少。
机制那一侧的综述解释了原因:吸烟让组织暂时缺氧,炎症细胞赶到伤口、杀灭细菌的能力变弱,修补用的成纤维细胞和胶原蛋白也跟不上。戒烟后供氧很快回来,炎症细胞约 4 周内部分恢复,修补那一步恢复得最慢。
所以有手术安排时,把抽烟的事告诉外科医生,并尽早开始戒,越早越好。

数字 · 骨头也记着这笔账
一项汇总 10 个前瞻性队列、59232 人的 (Kanis 2005)里,和不吸烟的人比,正在吸烟的人:任何骨折的约 1.25髋部骨折约 1.84
有意思的是只解释了一小部分:把骨密度算进去以后,髋部骨折的相对风险还有 1.60;骨密度偏低只占吸烟相关髋部骨折风险的 23%。也就是说,一次骨密度检查结果还行,并不代表抽烟对骨头的影响已经被看全了。

什么时候该看医生
When to see a doctor
抽烟的人,和家里有人抽烟的人,遇到下面这些情况不要在家等:
胸口疼,像被压住或攥紧,或疼痛往手臂、脖子、下巴走,伴出汗、喘不上气:可能是心脏病发作,立即拨打急救电话喘不上气到说不出话,或胸口、上背部疼:立即去急诊咳血,或咳进纸巾里看到血:尽快就医咳嗽超过 3 周;或者体重莫名下降、没胃口、总是很累、反复胸部感染:去看医生
这些症状大多数时候不是癌,但它们是让医生当面检查的理由,越早越好;40 岁以上的人尤其不要拖。
想戒却戒不掉,也值得专门为这件事看一次医生或戒烟门诊。

Adult smokers weigh on average 4-5 kg less than nonsmokers; smoking increases 24-h energy expenditure by about 10% (about 200 kcal/day); nicotine acts through central neurotransmitters on appetite-suppressing and metabolism-raising pathways (pro-opiomelanocortin, CART). Post-cessation weight gain comes from a fall in metabolic rate and a rise in intake: in one study quitters ate on average 227 kcal/day more, explaining 69% of the weight gained at 3 months. Cough is a vagal reflex; a poor cough reflex leads to recurrent aspiration; polymodal receptors such as TRPV1, TRPA1 and P2X3 on airway nerve endings respond to gastric fluid, acid, smoke, particulates and hyper- or hypotonicity, and ATP from stressed cells is an important stimulus; receptive fields also in lung parenchyma, pharynx, oesophagus and the ear canal (Arnold's reflex); cortical influences modulate the reflex and women have a greater somatosensory cortical area devoted to cough; the system shows redundancy, plasticity and adaptation; most patients show cough reflex hypersensitivity, triggered by low-level stimuli such as cold air, perfumes, smoke and bleach; specialist-clinic patients two-thirds female, most often in their sixth decade. T2 (eosinophilic) inflammation in about a quarter (cough variant asthma, eosinophilic bronchitis). Non-acid and gaseous reflux may contribute and oesophageal dysmotility is highly prevalent in chronic cough. ACE inhibitor cough in about 15%; ACE inhibitors reset the reflex so timing may not match; angiotensin II antagonists do not affect the cough reflex; no patient with cough should be given an ACE inhibitor; bisphosphonates and calcium channel antagonists may worsen reflux and cough; prostanoid eye drops such as latanoprost may descend the lacrimal duct and irritate the pharynx. Smoking is the major remediable cause; smokers have suppressed capsaicin cough sensitivity and cough may transiently increase in the first month after quitting. Stress urinary incontinence, interference with speech and depression are the most impactful complications; women with cough and incontinence have worse quality of life, and incontinence is rarely discussed, so it should be asked about. Initial assessment excludes malignancy, infection, foreign body and ACE inhibitor use; spirometry and chest X-ray; routine chest CT not suggested when X-ray and examination are normal (conditional, very low). Treatment is by sequential therapeutic trials, one agent at a time, stopped if no response; morphine responds within a week, ICS may take a month; if successful continue several months to let neuronal hypersensitivity resolve, then withdraw to test for remission. Short ICS trial 2-4 weeks (conditional, low): of ten RCTs, two in unselected chronic cough found benefit, while in non-asthmatic cough (negative methacholine) ICS was not superior to placebo. Short anti-leukotriene trial 2-4 weeks, particularly in asthmatic cough (conditional, low). Anti-acid drugs not routinely (conditional, low): no benefit over placebo without acid reflux, modest at most with it; high-dose esomeprazole for 12 weeks in patients with rare or no heartburn gave no benefit. Low-dose morphine in refractory cough (strong, moderate); gabapentin or pregabalin (conditional, low), limited by side effects such as dizziness, fatigue, cognitive changes, nausea and blurred vision; cough control therapy (physiotherapy, speech and language therapy) suggested (conditional, moderate).
