Saw palmetto is the best-selling prostate supplement, yet two large randomized trials, STEP and CAMUS, found it no better than placebo for the urinary symptoms of an enlarged prostate, and a Cochrane review of 32 trials reached the same conclusion. It is broadly safe, but it has been tested and showed no effect.
Two examples of one pattern: early small trials looked positive, and the effect disappeared once larger, stricter randomized trials were done. For saw palmetto that means the STEP and CAMUS trials plus an updated Cochrane review; for milk thistle, a Cochrane review that found no effect on death or complications.
