One way of eating does two jobs: the DASH diet lowered blood pressure in randomized trials, and in three large cohorts, people whose diets were closest to DASH had a 40–45% lower risk of kidney stones (an observed association). The citrate and potassium from fruit and vegetables may be part of why it means fewer stones.

Whether a blood vessel can relax depends on whether its lining makes enough nitric oxide (NO). Citrulline is converted in the body to arginine, which is then used to make NO: in a small crossover trial of 10 men, 6 g of citrulline a day for 7 days raised blood arginine as much as taking arginine itself, and mean arterial pressure fell somewhat.

In the DASH trial (Appel 1997), participants ate a diet rich in fruit, vegetables and low-fat dairy, and high in potassium, for 8 weeks, with sodium and body weight held constant; in those with hypertension, systolic pressure fell 11.4 mmHg more than on the control diet. In other words, eating more like DASH lowers blood pressure even without cutting salt; eating less sodium is a separate route.
Type 2 diabetes and high blood pressure often occur together and share mechanisms: insulin resistance, damage to the lining of blood vessels, an overactive sympathetic nervous system and an activated renin-angiotensin system; both are parts of the metabolic syndrome. People with diabetes are usually given a stricter blood pressure target than others (ADA 2024).
Sleep apnea is very common in people with resistant hypertension (blood pressure that stays uncontrolled on three drugs, including a diuretic), so these patients should be checked for it. Treating it with continuous positive airway pressure (CPAP) lowers blood pressure only a little on average, though some people see a larger drop.
The weightiest evidence that a dietary pattern changes blood pressure comes from feeding trials such as DASH: the researchers prepared all of the participants' meals (more fruit, vegetables, low-fat dairy and whole grains; less saturated fat and sweets), with sodium and body weight held constant, and blood pressure still fell (Appel 1997). The later DASH-Sodium trial showed that cutting sodium on top of the DASH diet lowered it further (Sacks 2001). How much each component contributes was not separated in these trials.
Aerobic exercise gives the most reliable drop in blood pressure in people whose pressure is high (Cornelissen 2013, pooling 93 trials). Isometric exercise works too, such as squeezing a handgrip or holding a wall sit: Smart 2019, pooling each participant's data across trials, found systolic pressure fell by about 6.9 mmHg and diastolic by about 3.9 mmHg on average, similar to the average effect of a single blood-pressure drug.
The enzyme that ACE inhibitor blood pressure drugs block also normally breaks down bradykinin and substance P; when these build up in the airway they turn the cough reflex up, and about 15% of people on these drugs develop a dry cough, sometimes months after starting. Stopping the drug is the only reliably effective fix, usually easing within 1 to 4 weeks; angiotensin receptor blockers do not affect the cough reflex.
The rise in cardiovascular risk after menopause is real, but it adds to the conventional risk factors, and those you can manage. Blood pressure is the highest-return and most measurable of them: check it regularly and keep it within your target.
The small-vessel route is the most insidious: years of high blood pressure thicken the walls of the tiny penetrating arteries deep in the brain and narrow them until they close, leaving small lacunar infarcts behind. Any one may cause no symptoms, but as they accumulate they show up as unsteady walking, slower reactions and cognitive decline.
In an analysis pooling 15 heat-therapy studies, several weeks of repeated heat lowered systolic and diastolic blood pressure by about 3.9 mmHg each on average. The studies covered not only sauna but also hot-water immersion; the effect is modest, and it is evidence of blood-vessel adaptation, not a prescription to treat high blood pressure with a sauna. People who already have high blood pressure should still follow their doctor.
Alcohol is one of the reversible causes of high blood pressure: the more you drink, the higher your blood pressure tends to be, and cutting back or stopping brings it down somewhat within weeks. It is one of the few blood pressure measures that shows results quickly.
Bacteria on the surface of the tongue reduce dietary nitrate to nitrite, which the body then turns into nitric oxide, and the blood vessel walls relax a little. Siervo 2013, pooling 16 trials, found systolic blood pressure fell by about 4.4 mmHg on average: a modest, mostly short-term effect, one piece of a diet rather than a blood pressure prescription.

Controlling blood pressure guards this door, and it and lowering blood lipids are two jobs that both need doing.

High blood pressure, smoking and high blood glucose all injure that lining and make it easier for particles to get in. Libby 2011 describes atherosclerosis as an inflammatory process in the artery wall, not just an LDL cholesterol number on a lab report. Controlling blood pressure protects this door; how many particles there are is still managed by lowering blood lipids.
Evidence · What these two sources say
Libby 2011 is a review of the biology and Grundy 2019 is a cholesterol management guideline, and neither gives a figure for how much plaque blood pressure control prevents. Do not carry results from blood pressure drug trials straight over to this question. What you can take away is the direction: blood pressure injures the lining so particles get in more easily; lowering the number of particles and protecting the lining are two separate doors, and one cannot replace the other.
Clinical · Two measures, two jobs
Controlling blood pressure does not replace lowering ApoB: even with the door guarded, how many particles are pushing at it is still set by blood lipids. Grundy 2019 is a lipid guideline, and blood pressure is another item in the same assessment of atherosclerotic cardiovascular disease risk. Nor should high blood pressure be diagnosed backward from a lipid test. Each has its own diagnostic criteria and risk levels, and they cannot be merged into one all-purpose checkup sheet.
