孩子长高 · 骨头怎么变长、什么真有用
孩子长高靠骨头两头的生长板,睡眠、吃够和青春期早晚都在影响它。这里讲什么真有用、增高产品为什么靠不住,以及哪些情况要看医生。

骨头在生长板上变长

孩子长高,是腿骨和脊柱里的骨头一点点变长;而骨头变长只发生在一个地方:骨头两头的生长板。
生长板是长骨两端夹着的一层软骨。里面的软骨细胞先分裂,排成一摞一摞,再把自己撑大,这些软骨随后被骨头替换,骨头就往外多出一截(Baron 2015)。身高的增加,就是这层软骨不停地造出来、又不停地变成骨头。
这层软骨不会一直开工。它里面能分裂的储备细胞次数有限,越用越慢,所以孩子越大长得越慢;到青春期末,整层软骨被骨头完全取代,叫骨骺闭合,从此骨头不再变长(Nilsson 2004)。
所以孩子还能长多少,取决于两件事:生长板还开着多久,以及这段时间里它干活的条件好不好。后面几章讲的就是这些条件。孩子的身高在生长曲线上一路往下掉,或者比同龄人矮得多,什么时候该带孩子看医生那一章列了要看医生的情况。

机制 · 软骨怎么变成骨头
在显微镜下,生长板从骨头末端到骨干分成三层(Baron 2015):静止层:一批安静的储备细胞,是整片生长板的种子。增殖层:细胞开始分裂,新细胞沿着骨头的长轴排成一根根柱子。骨头是变长而不是变粗,靠的就是这些柱子的方向。肥大层:细胞停止分裂,把自己撑大,同时分泌软骨基质。
撑大的软骨随后被血管和造骨细胞侵入,改造成真正的骨头,这个过程叫软骨内成骨(endochondral ossification)。
骨头为什么越长越慢?一种解释是,静止层的储备细胞能分裂的次数有限,用掉一批就少一批,生长板于是自己慢慢老化;这套解释的证据主要来自动物实验(Nilsson 2004)。生长板一旦闭合,软骨就没有了,所以闭合之后再吃什么、做什么,骨头都不会再变长。

生长激素跟着深睡走
Growth hormone follows deep sleep
推动生长板干活的头号信号是生长激素。它一天里最大的一次分泌,跟着入睡后的第一段深睡走,不跟着钟点走。
生长激素由大脑底部的垂体分泌,是一阵一阵往外放的。它一部分直接作用在生长板上,一部分让肝脏造出另一种生长信号,叫胰岛素样生长因子 1(),随血流到达生长板;生长板自己也会就地造一些 IGF-1(Baron 2015)。
把生长激素和睡眠连起来的,是两项在年轻成人身上做的小研究:生长激素的高峰在进入深睡时出现,推迟入睡,高峰也跟着推迟(Takahashi 1968);把睡觉和清醒的时间整个颠倒 12 小时,高峰也跟着睡眠走(Sassin 1969)。
按这个机制推,对长个子要紧的是睡够、睡得深,而不是晚上十点前必须睡着这样的钟点;睡眠和最终身高之间的这一步,在孩子身上没有被直接测过。

证据 · 这两项睡眠研究测了什么
Takahashi 1968 给 8 个年轻成人一共做了 38 个晚上的监测:每 30 分钟抽一次血,同时记录脑电和眼动。8 个人里有 7 个,在进入深睡时出现一次持续 1.5 到 3.5 小时的生长激素高峰,之后的深睡里偶尔还有小高峰。入睡推迟,高峰跟着推迟;被叫醒 2 到 3 小时再睡回去,又出现一次高峰。这个高峰和血糖、胰岛素、皮质醇的变化都没有关系。
Sassin 1969 把受试者睡觉和清醒的时间整个颠倒 12 小时,睡眠里的生长激素分泌也跟着颠倒。它说明这次分泌不是按一天里的钟点自己走,而是跟着睡眠,尤其是脑电波最慢的慢波睡眠,也就是深睡。

父母身高给的是范围
Parents' heights give a range
孩子最后长多高,最大的一份由基因决定;但父母的身高给的只是一个范围,不是一个数。
一项汇总 20 个国家、45 个双胞胎队列的分析发现,越小的孩子,一家人共有的环境(吃什么、生过什么病)对身高差异的影响越大;年龄越大,基因的份量越重,到青春期最高,能解释男孩约 83%、女孩约 76% 的差异(Jelenkovic 2016)。这说的是一群孩子之间为什么高矮不同,不是某一个孩子的身高有多少是天定的。管身高的基因有几百个,大多数不在生长激素这条线上,而是直接管生长板里的软骨细胞(Baron 2015)。
医生会用父母的身高推出孩子的遗传靶身高。英国儿科学会的生长曲线写明:每 5 个孩子里约有 4 个,成年身高落在这个靶身高上下 7 厘米以内。
所以父母不高,孩子未必矮;但孩子的身高如果明显低于父母给出的范围,就值得请医生看一看。

实操 · 怎么读孩子的生长曲线
生长曲线上的每条线叫一个百分位。孩子在第 50 百分位,意思是 100 个同龄同性别的孩子里,他大约排在正中间;在第 2 百分位,大约只有 2 个孩子比他矮。一格:两条相邻百分位线之间的距离。健康孩子的身高大多沿着自己那一格走,不会一路穿过好几条线。和父母比:把父母的身高点在曲线旁的标尺上,连一条线,得到孩子的父母身高百分位。英国的生长曲线写明,10 个孩子里有 9 个,身高百分位落在它上下两格以内,只有约 1% 的孩子会低于它三格以上。量准:脱鞋、靠墙站直,用固定的身高尺量;一次的数字说明不了什么,几次连起来看趋势才有意义。
这套方法来自英国的生长曲线(RCPCH)和英国儿科内分泌学会的标准。不同国家用自己的生长曲线,看百分位、看趋势的道理是一样的。

吃够了才长得动
Eating enough to grow
营养对身高更像一道门槛:吃不够,生长板就慢下来;吃够了以后,再多吃也不会让骨头长得更长。
吃不够的孩子,血里的生长激素往往不低,甚至偏高,长得却慢:缺能量和蛋白质时, 等生长信号被调低,有限的原料先用在维持生命上(Baron 2015)。锌也有类似的门槛:汇总 33 项随机试验,补锌让孩子长高得多一些,效果主要出现在原本偏矮、偏瘦、可能缺锌的孩子身上(Brown 2002)。
钙和维生素 D 管的主要是骨头够不够硬。缺得厉害会得佝偻病,生长板钙化不了,腿会弯、手腕和脚踝变粗、长得慢(Munns 2016)。但补钙多出来的是骨量,不是身高:一项在平时钙吃得很少的中国 7 岁孩子里做的随机试验,补钙 18 个月,骨矿含量涨得更多,身高增长没有差别(Lee 1994)。牛奶对身高的作用也有试验测过,幅度很小(Hidayat 2023)。
所以要做的是吃够、吃得杂;要不要补锌、补钙,查过再听医生的。

证据 · 补锌、补钙、喝奶各测出多少
锌(Brown 2002):33 项随机试验的 ,补锌对身高增长的效应量是 0.35 个标准差。越是本来偏瘦、6 个月以上又偏矮的孩子,效果越明显。作者的建议针对的是缺锌风险高、生长迟缓常见的人群,不是所有孩子。钙(Lee 1994):162 个平时钙吃得很少的 7 岁中国孩子,随机分到钙片组和安慰剂组。18 个月后,钙片组前臂桡骨的骨矿含量涨了 16.5%,对照组涨了 13.97%;身高增长两组没有差别。乳制品(de Beer 2012):12 项对照试验的汇总,最可能的效应是每天多喝约 245 毫升牛奶,每年多长约 0.4 厘米;原本偏矮的孩子和青少年效果更大。但很多试验没有随机分组、年代也久,作者把证据评为中等质量。乳制品(Hidayat 2023):较新的随机试验汇总,看了身高的有 15 项,喝奶组身高多 0.21 厘米,血里的 也升高了。作者认为这和蛋白质促进合成的作用一致,也说骨量上的改善幅度很小。放在一起看:钙片加的是骨量;牛奶带着蛋白质,能稍稍推动 IGF-1 和身高;锌在缺锌的孩子身上才看得出效果。这些试验里的增益都不大,而且集中在原本吃不够、长得慢的孩子身上。

青春期冲刺与生长板闭合

青春期是孩子长得最快的一段,也是生长板开始关门的一段。让它关门的是雌激素,男孩女孩都一样。
美国孩子的数据里,女孩平均约 9 岁、男孩约 11 岁开始加速;整个青春期,女孩约长 27.5 到 29 厘米,男孩约长 30 到 31 厘米(Abbassi 1998)。
男孩体内也有雌激素,是睾酮在身体里转化来的。一个雌激素受体失灵的男人,青春期发育正常,28 岁时还在长高,身高 204 厘米,生长板没有闭合(Smith 1994)。这类罕见病例说明,生长板最后关门要靠雌激素;雌激素会加快生长板里储备细胞的消耗,让它提早老化(Baron 2015)。
所以青春期来得太早,生长板也会提早关门,最终身高可能吃亏。女孩 8 岁前、男孩 9 岁前出现青春期迹象(乳房发育、阴毛、睾丸变大),要带去看医生(Latronico 2016)。

证据 · 早发育和晚发育谁更高
Abbassi 1998 汇总的美国数据里,长得最快的那一年,女孩约在 11.5 岁,一年长约 8.3 厘米;男孩约在 13.5 岁,一年长约 9.5 厘米。起跳早的孩子,青春期里长得更多;起跳晚的,青春期里长得少一些。但起跳年龄和最终身高没有关联。所以在正常范围内,早几年蹿个子的孩子最后不一定更高,晚发育的孩子也常常只是晚到,不是长不高。
正常范围之外是另一回事。Morishima 1995 报告的一对姐弟不能把雄激素转成雌激素,弟弟 24 岁时骨龄只有 14 岁,身高 204 厘米:没有雌激素,生长板就迟迟不关。反过来,青春期来得太早,生长板也就关得早。
有人想给发育时间正常的孩子打抑制青春期的针,好多长几厘米。一份欧美专家共识写明:这类药(GnRH 类似物)能提高成年身高,只在很早开始的中枢性性早熟(女孩 6 岁前)里是确定的;用在其他情况还需要研究,不能常规建议(Carel 2009)。要不要用,由儿童内分泌医生判断。

增高产品和常见说法

增高药、增高保健品、增高鞋垫,都没有可靠的试验证明能让孩子最终长得更高;能让孩子长高的,还是前面几章那几件事。
增高保健品:成分多是钙、锌和各种氨基酸。按前面讲的,补钙加的是骨量;锌只在缺锌的孩子身上看得出效果,缺不缺要查过才知道。号称含生长激素的口服品:生长激素是一种蛋白质,吃下去会在肠胃里像别的蛋白质一样被拆开,所以医生用它只能打针。来路不明的增高药:如果偷偷加了性激素,按机制推反而会让生长板提早关门;这一步没有被直接测过。拉伸、吊单杠、增高鞋垫:人一天里本来就有高有矮。一项研究里 8 个年轻成人早晚相差约 19.3 毫米,一半多在起床后第一个小时就缩掉了,夜里躺下又回来(Tyrrell 1985)。那是脊柱里的椎间盘被压扁又回弹,骨头没有变长;鞋垫只是垫高了鞋。举铁会长不高:在儿童力量训练的对照试验里,身高增长没有受影响(Malina 2006),青少年训练那一篇有细讲。

临床 · 生长激素针是给谁用的
生长激素针是真药,用在查明了原因的生长问题上,比如身体自己造不出足够的生长激素;用不用、怎么用,由儿童内分泌医生检查后决定。有的孩子只是个子矮,查不出原因,叫特发性矮小。一篇系统综述汇总了这类孩子的 3 项随机试验,共 115 个孩子:打生长激素的孩子成年后平均比对照组高约 4 厘米;另外 7 项非随机试验给出的是约 3 厘米。作者说,这比它在获批的那些病上的效果小,而且每个孩子的反应差别很大(Deodati 2011)。
所以它不是给健康孩子多长几厘米的捷径。孩子是不是真的矮、有没有病因,先按什么时候该带孩子看医生那一章的标准去看医生;查不出病因的矮小孩子用不用这类药,是要和儿童内分泌医生一起权衡的决定。

什么时候该带孩子看医生

多数个子偏矮的孩子是健康的:家里人本来就不高,或者只是发育得晚。下面这些情况,值得带孩子去看儿科或儿童内分泌科。
看身高本身:
身高低于同龄同性别孩子的第 0.4 百分位,也就是 1000 个孩子里最矮的那 4 个。身高的百分位比父母身高推出的百分位低 3 格以上。身高在曲线上往下掉,掉过了 1 格以上(先排除量错)。还没到青春期,身高却突然往上蹿,越过百分位线往上走。
看青春期时间:

Nutritional rickets is a disorder of defective chondrocyte differentiation and mineralization of the growth plate and defective osteoid mineralization, caused by vitamin D and/or calcium deficiency; it affects health, growth and development. Signs include swelling of wrists and ankles, leg deformity (bow legs or knock knees), rachitic rosary, delayed fontanelle closure, hypocalcemic seizures and failure to thrive. The supplemented group gained significantly more radial bone mineral content (16.5% vs 13.97%) and BMC/bone width than controls, but there was no effect on height increment. Meta-analysis of 21 publications of randomized trials. Dairy supplementation slightly increased whole-body and regional bone mineral content and density and height (0.21 cm, 95% CI 0.09-0.34; 15 trials), raised serum IGF-I and lowered parathyroid hormone. Mean take-off age of the pubertal spurt is about 11 years in boys and 9 years in girls; peak height velocity comes at a mean 13.5 and 11.5 years, with whole-year peak velocity of 9.5 cm/y in boys and 8.3 cm/y in girls. Pubertal growth contributes about 30-31 cm in boys (17-18% of final height) and 27.5-29 cm in girls (17%). Few controlled prospective studies exist and many conclusions rely on expert opinion. - Malina, R. M. (2006). Weight training in youth — growth, maturation, and safety: an evidence-based review. Clinical Journal of Sport Medicine, 16(6), 478-487. Review of 22 experimental training studies in pre- and early-pubertal youth, mostly 8- and 12-week programmes: strength rose and was lost with detraining; the programmes did not influence growth in height and weight; only 10 studies monitored injuries (3 reported). Supervised programmes with low instructor-to-participant ratios were relatively safe. - British Society for Paediatric Endocrinology and Diabetes, Growth Disorders Special Interest Group. (2021). Clinical standards for growth assessment and referral criteria for children with a suspected growth disorder. UK guidance recommends referral for height below the 0.4th centile (-2.67 SDS). Additional criteria for growth assessment: height centile more than 3 centile spaces (>2.0 SDS) below the mid-parental centile and/or a drop in height of more than 1 centile space. Red flags: weight loss, constipation or diarrhoea, headaches, vomiting, abnormal fat stores, disproportion, dysmorphic features, anaemia, delayed puberty, clinical features of hypothyroidism or other chronic illness. Tall stature or accelerated growth (height >98th centile, or an increase of 1 centile space over more than a year before puberty) also merits assessment. Referral is recommended for girls with no signs of puberty by 13 and boys by 14.
