心慌 · 早搏 · 听见自己心跳的时候
心慌是察觉到了自己的心跳,多数不危险但会反复。漏一拍多半是早搏后更有力的那一下;突然开始又突然停的飞快心跳,要在发作时抓住。伴胸痛、气短或晕倒立即急诊。

心慌是什么,危险吗

心慌(医学上叫心悸),说的是你察觉到了自己的心跳。心脏一直在跳,平时你感觉不到;心慌的时候,它变得很快、很重、不齐,或者像漏了一拍。这种感觉可能在胸口,也可能在脖子或喉咙里。
它很常见,多数不危险。一项研究连续追踪了 190 位因为心慌去看病的人:43% 的原因在心脏,31% 是焦虑等心理原因,10% 是别的原因,16% 查不出来。一年后,死亡的占 1.6%,卒中的占 1.1%;但 75% 的人这一年里又心慌过。
所以这一篇不是要吓你,而是讲两件事:哪种心慌要马上处理,以及怎样在发作的那一刻把它抓住,让医生看清是哪一种。

证据 · 有焦虑,也要先看心脏
在那 190 个人里,40% 的原因靠问诊、查体、一张心电图或几项化验就找到了。焦虑是心脏以外最常见的原因,但有焦虑不等于心慌就是焦虑引起的:在心理原因占主导的人里,仍有多达 13% 查出了别的原因。所以医生的做法是:每个来看心慌的人,都先看有没有心脏方面的原因。
有两个特点让心律问题的可能小一些:每次心慌不到五分钟,以及家里有人有惊恐障碍。它们只是让可能性变小,不能用来排除。
第 2 章
漏一拍,其实是后面那一下

心脏每跳一下,都由右心房上的窦房结先发出一个电信号,顺着心房传到房室结,再沿着传导束散到两个心室。偶尔,心房或心室里别的地方抢先放了一次电,心脏就提前跳了一下,这叫早搏。
奇怪的是,你感觉到的往往不是早搏本身。单个早搏之后紧跟的那一下心跳会格外有力,很多人说的漏了一拍或者咚地一下,感觉到的其实是这一下。
早搏极其常见:长时间戴着心电记录,大多数人都能查到室性早搏。年纪大、个子高、血压高、有心脏病、活动少、吸烟的人,早搏更多。
察觉得到察觉不到,和早搏多不多不完全是一回事。久坐、焦虑或情绪低落的人更容易察觉,常活动、心情好的人反而不太注意。早搏也常在安静下来的时候被发现。
奇怪的是,你感觉到的往往不是早搏本身。单个早搏之后紧跟的那一下心跳会格外有力,很多人说的漏了一拍或者咚地一下,感觉到的其实是这一下。

察觉得到察觉不到,和早搏多不多不完全是一回事。久坐、焦虑或情绪低落的人更容易察觉,常活动、心情好的人反而不太注意。早搏也常在安静下来的时候被发现。
机制 · 什么样的早搏要多查一步
心房早搏和心室早搏都很常见,有没有心脏病的人身上都有。单个、形态都一样的室性早搏,更可能是良性的;成串出现、形态多样的,要多看一眼。对室性早搏,医生要回答两个问题:
心脏结构有没有问题:通常做一次心脏超声。有结构性心脏病的人,早搏的意义不一样,要先治那个病。早搏占全部心跳的比例:要戴着心电记录数出来。比例到 25% 以上,心肌被拖累、变弱的风险会升高。
心脏结构正常、早搏不多、症状又轻的人,医生多半会告诉你它是什么、不用治。症状重的,可以用药,或者做导管消融。
第 3 章
咖啡、酒和熬夜

很多人一心慌就先戒咖啡。2023 年一项试验给 100 个成年人戴上连续心电记录 14 天,每天用短信随机告诉他们今天喝咖啡还是不碰咖啡因:
房性早搏:喝咖啡的日子每天 58 次,不喝 53 次,差别不显著。室性早搏:每天 154 次对 102 次,多了大约一半。喝咖啡的日子多走了 1058 步,夜里却少睡了 36 分钟。
另一项研究看了 1388 位平均 72 岁的老人:平时喝多少咖啡、茶,吃多少巧克力,和 24 小时里的早搏数都没有关系。
合起来看,咖啡不是心慌的统一开关。它可能让一部分人的室性早搏短期变多,也会挤掉睡眠,而睡不够本身就是常见的诱因。值得自己观察,不必一刀切。
其它常见诱因还有剧烈运动、压力和焦虑、酒、尼古丁和毒品。有些药也会,比如治哮喘的缓解吸入剂、治多动症的药,看一眼说明书。更年期和怀孕期间也常有心慌。

实操 · 给自己做一次小试验
怀疑是咖啡,可以借那项试验的设计:喝和不喝的日子穿插着来,同时记下每天心慌了几次。这比凭印象准,因为心慌本来就时多时少。这是从试验设计借来的办法,不是指南的建议。没有查出疾病的心慌,少碰诱因本身就有帮助:压力、吸烟、咖啡因和酒。
在吃 β 受体阻滞剂(比如美托洛尔)的人,别自己突然停药:突然停药本身就会引起心慌。想减量,先和开药的医生商量。
第 4 章
突然飞快,又突然停下

另一种心慌是:心跳突然飞快、很齐,过一阵又突然停下,像有人开关了一下。这常是阵发性室上性心动过速:电信号在心脏里的一个回路中打转,每转一圈,心脏就跳一下。
它和惊恐发作很像,所以常被认错。一项研究回看了 107 位确诊这种心动过速的人:
67% 的症状符合惊恐障碍的诊断标准。55% 在第一次看病时没被认出来,中位数拖了 3.3 年。这些人里,54% 被非精神科医生归到惊恐、焦虑或压力上;女性比男性更常被这样归因(65% 对 32%)。治疗(多数是导管消融)后,86% 的人症状消失了。
两者的差别常在起止方式:心动过速多是突然开始、突然停;焦虑带来的心慌多是慢慢加快,心跳整齐、只是稍快,还常伴着手和脸发麻。
如果你的心慌是开关式的,看医生时把这一点说清楚。

实操 · 确诊之后能学的一个动作
迷走神经能让心跳慢下来,有一类动作就是去刺激它,其中最常用的是 Valsalva 动作(憋住气用力)。医生可以教确诊过室上速的人自己做。英国急诊的一项试验比较了两种做法:
标准做法:半躺着憋气用力 15 秒(试验里用压力计控制在 40 mmHg),1 分钟内恢复正常心律的占 17%。改良做法:同样用力完,立刻平躺,由别人把双腿抬高,这个比例到了 43%,没有出现严重不良事件。
这个动作要在医生确诊、教过之后再自己用。没确诊过的心慌,不要拿它当自测;发作时伴胸痛、气短或要晕倒,直接去急诊。偶尔发作、能自己停的,不治疗也是一个选项,和医生商量。

在发作时抓住它
Catch it while it happens
心慌最难办的地方是:等你到了医生面前,它往往已经停了。不发作时做的心电图正常,排除不了心律问题。所以关键是在发作的那一刻留下记录:
当场摸脉搏,或者用手指把节奏敲出来:快还是慢,齐还是乱。记下来:突然开始还是慢慢开始,持续多久,当时在做什么,有没有胸痛、气短、头晕。手表能记心电图的,发作一开始就记一段。一项研究教 59 位心慌的人在症状出现时用手表记 30 秒单导联心电图,收到的 77 段里 95% 能看清,78% 看到了心律失常。
医院的检查也按发作频率选:几乎天天发作的,戴 24 到 48 小时的动态心电图;不常发作的,要戴能记约 30 天的事件记录仪。前面那项室上速研究里,动态心电图只抓到 9%,事件记录仪抓到 47%。

临床 · 手表报警和该查的血
手表提示脉搏不规则怎么办?一项 41.9 万人的研究里,收到提示的只有 0.52%;其中后来贴上心电贴片的人(平均在 13 天后),34% 查到了房颤。别忽视它,也别被它吓坏,去做一次心电图。房颤的心跳是毫无规律的乱,它会让卒中风险升高。身体本身的问题也会让心跳变快或不齐:甲亢、贫血、低血糖、血钾或血镁异常、怀孕。站起来就心慌的,要量一量躺着和站着的血压、心率。基层常做的化验是和肾功能、电解质;新发现房颤或有甲亢症状的,再查甲状腺功能。
这些靠抽血和测量来查,不是先吃一片镁或钾能筛出来的。

什么时候必须就医
When to see a doctor
立即去急诊(别自己开车,叫救护车或请人送):
心慌一直不停。心慌伴着胸痛、喘不上气、头晕要晕倒或已经晕倒。
当天联系医生:
上面这些症状已经停了。一运动就心慌。新出现的心跳乱、毫无规律。安静时心率超过每分钟 120 次,或低于 45 次(发作还在持续,按上面去急诊)。
尽快看医生,做一次心电图:
心慌反复出现、越来越频繁,或者一次持续超过几分钟。本来就有心脏病;家里有人有心脏病,或者有人年纪轻轻就猝死。
去的时候,把在吃的药都带上,也把你记下的发作情况带上。
