Fire first, blood follows

The brain has no fuel tank. The moment a neuron fires, ion pumps start burning adenosine triphosphate: The cell's universal energy currency — almost everything that costs energy spends it., and sugar and oxygen have to arrive in seconds. The body does not pressurize the whole head. It opens the capillaries next to the patch that just fired.
Firing raises local carbon dioxide, potassium and adenosine. Those messengers tell the nearby tubes: work is happening here, open a little. Blood brings glucose and oxygen; waste is flushed.
So the bright patch on functional MRI is blood, not electricity. Electricity is milliseconds; the blood response lags. What you see is fuel matching activity, not a brain blushing from injury.

Mechanism · a match, not a tank
If the brain were a sponge sitting in a lake of blood, it would not matter which cluster was firing — sugar would seep in anyway. The opposite is true: the capillary net follows activity. Attwell 2010 lays the chain out — messengers from active neurons change vessel diameter via glia and pericytes, and local blood flow tracks the work.That is why concentrating on a sum does not turn the whole head redder. Blood is turned up next to that calculation path, then turned back down when the path goes quiet.

Who turns the tap

The neuron does not turn the tap. The cells next door do.
Astrocytes send end-feet that wrap the capillary. Glutamate and potassium from the firing neuron are received by glia first, which then tell the tube to open or close. Pericytes sit on the capillary wall and can narrow or relax the diameter — a large share of flow at this smallest level is theirs.
Endothelium welds the three together. Iadecola calls the assembly a neurovascular unit: neuron, glia, pericyte and endothelium are not four neighbours. They are four parts of one matching machine.

Not the migraine dilator
The migraine island on this site is the trigeminal fibre releasing CGRP, meningeal vessels opening, pain turning up. That is a pain neurovascular chain.Coupling here is a different job: a working cortical neuron asking the capillary under its own feet for sugar. No CGRP, and not via the meninges. Two sets of vessels, two messengers. Do not collapse them into 'vessels in the brain dilate'.

The bright patch is blood

The BOLD signal used in fMRI reads how local deoxyhaemoglobin changes. When neurons work, incoming fresh blood often exceeds the oxygen they burn at once, oxygenated haemoglobin rises, and the scanner paints the patch bright.
So that colour map is a blood-supply map, one inferential step away from electrical activity. The electricity already happened; the blood is still arriving. The scanner is not lying — coupling itself has a lag. When you read the picture, remember: you are seeing whether the match occurred, not whether a synapse fired.

When the match dulls

Hold small arteries at too high a pressure for years and the wall thickens and stiffens. The tap is still there; it turns sluggishly. When a patch should open, it opens late. The thin cables of white matter feel ischaemia first, and conduction loses half a beat.
This is not a second incidence table. It is the other face of the same mechanism: coupling depends on a wall that can still hear messengers. When the wall cannot hear, the fuel match dulls. On the cognitive-aging island, what slows is white-matter insulation — one road into that sentence starts here. What is kind to the heart's small vessels is often what leaves this match some slack.
