Electrons pass along the complexes of the inner mitochondrial membrane to O₂, pumping H⁺ across the membrane and building the proton gradient that drives synthesis.The inner mitochondrial membrane has 5 complexes (I-V). Electrons from /FADH₂ flow from I/II through III to IV, finally reducing O₂ to water. Electron flow pumps H⁺ across the membrane, creating the proton gradient that drives synthesis.
2 · CoQ10 · the electron shuttle
Coenzyme moves freely within the inner membrane, taking electrons from Complexes I and II to Complex III; without it, the chain breaks there.Coenzyme (ubiquinone) is a lipid-soluble molecule embedded in the inner membrane lipid bilayer. Its job: accept electrons from Complexes I and II, carry them to Complex III. It is the only freely diffusible electron carrier in the membrane — without it, the ETC breaks between I/II → III. The reduced form is ubiquinol.
3 · Dual role · antioxidant
also scavenges free radicals in the inner membrane, and its reduced form regenerates vitamin E so the same molecule can quench again and again. also directly scavenges free radicals, especially peroxyl radicals in lipid peroxidation chains. It acts in situ at the inner mitochondrial membrane (where production is highest). The direction is one-way: reduced CoQ10 (ubiquinol) regenerates the tocopheroxyl radical, letting the same vitamin E molecule quench again and again; CoQ10 itself is re-reduced by the respiratory chain (complexes I/II), with ascorbate able to assist. It is not a mutual cycle — CoQ10 sits upstream of vitamin E. This is the rationale for CoQ10 supplementation with statins, which deplete CoQ10.
4 · Supplementation · who benefits
is needed in primary deficiency, one heart-failure trial saw better outcomes, statin muscle-pain trials were negative, and in healthy people the evidence is weak.Statin users: statins inhibit HMG-CoA reductase, reducing synthesis; but a of randomized trials found no benefit of CoQ10 for statin-associated muscle pain, and a trial that confirmed the pain under blinding was negative too. Heart failure: the Q-SYMBIO randomized trial showed improved outcomes (420 people, a modest size; moderate certainty, not yet independently replicated). Primary CoQ10 deficiency: rare but essential. Healthy anti-aging/energy: weak evidence, not routinely recommended.
