is set by the weakest of 4 serial links, lung, heart, blood and muscle, and the main bottleneck is cardiac output. = maximum oxygen uptake per unit time (mL O₂/kg/min). It's not a single-organ metric — it's decided by the weakest of 4 serial links:· Lung (gas exchange) — healthy people rarely bottleneck here unless they have lung disease
· Heart (cardiac output Q) — Bassett & Howley 2000 review: ~ 70-85% of VO2max individual variance comes from cardiac output, this is the main bottleneck
· Blood (haemoglobin + O₂ carrying) — iron deficiency / altitude adaptation / EPO all operate here
· Muscle (mitochondria + capillaries) — low-to-moderate endurance training (Zone 2, for example) works mainly here, and it shapes submaximal pace more than VO2max itself
Key insight: the VO2max ceiling is set by the heart. That's why:
· Sustained endurance training makes each heartbeat push out more blood → VO2max climbs slowly
· But how much it rises varies from person to person: in the HERITAGE study, the same training added about 400 mL/min on average, and some people barely improved (Bouchard 1999). Most people cannot train up to the level of elite endurance athletes.
