Degeneration on a scan is neither necessary nor sufficient for back pain: disc degeneration and bulges are extremely common in people with no symptoms, and whether the nervous system is turning pain up or down does not show on imaging. So major guidelines do not recommend routine imaging for ordinary back pain without red-flag symptoms; first-line care is a clear explanation and staying active.
Modern care for acute soft-tissue injuries has replaced complete rest with appropriate loading: long bed rest or keeping an arm in a sling both weaken the muscles that should stabilize the joint and stiffen the tissues that should glide, so recovery ends up slower.
