The Toulis 2010 meta-analysis pooled 4 randomized trials (most patients were already on levothyroxine): after 3 months of selenium, anti-thyroid peroxidase (TPO) antibody titers were lower; effects on thyroid function, dose needs and thyroid structure were either not seen or not reported. Selenium has a U-shaped dose response and too much is harmful, so do not megadose.

The thyroid needs iodine to make hormone, but people with Hashimoto's thyroiditis are more sensitive to iodine than others: long-term excess iodine is linked with worsening thyroid autoimmunity, while too little means too little hormone. The aim is enough without excess, not as much as possible; do not eat large amounts of high-iodine foods such as kelp every day.
Hashimoto's thyroiditis is an autoimmune disease in which the immune system attacks the thyroid, and one of the most common causes of an underactive thyroid; it plays out along the hormone axis of hypothalamus, pituitary and thyroid. Seen along that axis, it becomes clear why thyroid-stimulating hormone (TSH) is the test to watch, and why levothyroxine is taken on an empty stomach and kept apart from acid-suppressing drugs and iron supplements.
An underactive thyroid brings tiredness, loss of interest, weight gain and slowed thinking, which look much like depression. So checking thyroid-stimulating hormone (TSH) before treating depression is routine practice; depression is also more common in people with Hashimoto's thyroiditis than in others (an observed association).
With an underactive thyroid, feeling the cold more than usual is one of the common symptoms, often alongside extreme tiredness, constipation and weight gain. For someone who has only recently started to feel unusually cold, a thyroid blood test is a cheap step.
Thyroid hormone influences the hair growth cycle. With an underactive thyroid (hypothyroidism), hair often thins all over and turns dry and brittle, usually alongside fatigue and feeling cold. This kind of hair loss shows up on a blood test and usually recovers with treatment, so it is worth ruling out early.
An underactive thyroid is a routine item in a fatigue work-up: once found, it is corrected with levothyroxine, which makes it a reversible cause rather than something to be written off as chronic fatigue.
