Macrophages patrol the body — when they meet bacteria / virus / fungi, they recognise pathogen-associated molecular patterns (PAMP) via Toll-like receptors (TLR).TLR activates downstream signalling →· Induces the cell to synthesise its own CYP27B1 → the macrophage locally converts into 1,25(OH)₂D (kidney-independent)
· Upregulates (vitamin D receptor) expression
This local production + local use model is the so-called immune paracrine mechanism — it bypasses the tightly regulated systemic circulation to act immediately at the infection site.
This is also why in severe D deficiency, even when systemic calcium metabolism barely copes, the immune system collapses first — it depends on local saturation of the D pool.
