Stomach acid and pepsin break down food protein to release the B12 bound to it, and in people short of stomach acid this step collapses.Food B12 is tightly bound to animal protein. Stomach acid (HCl, pH ~ 1.5) plus pepsin hydrolyzes the protein → B12 is released and exposed in gastric juice.Hidden trap: this step collapses when gastric acid is inadequate — long-term users, older adults with atrophic gastritis (~15-20% in the 60+ population), post-gastric-bypass / gastrectomy patients — no matter how much B12 they eat, it cannot be released.
