Hypertension, hyperglycemia, smoke and oxidative stress repeatedly injure the endothelium so it makes less , and this endothelial dysfunction comes before rising and plaque.Atherosclerosis is not a one-off event — it is decades of chronic inflammation.Starting point: chronic hypertension / hyperglycemia / smoke / oxidative stress repeatedly punctures the endothelium's tolerance. The earliest reversible marker: production ↓ → 'endothelial dysfunction'. This step is earlier than rising , earlier than plaque, earlier than any symptom.
· Smokers: smoke oxidants quench NO, so less NO is available at the endothelium
· Diabetics: chronic hyperglycemia uncouples eNOS, producing less NO than baseline
