Long COVID means multi-system symptoms that appear or persist for weeks after acute infection, and it splits into branches such as cardiovascular, neurocognitive, respiratory and PEM.Long COVID (PASC, Post-Acute Sequelae of SARS-CoV-2) is the largest new chronic-disease wave since 2020.Definition (WHO 2021 + NICE 2021)
· Symptoms new / persistent / relapsing 4 weeks after acute COVID
· Lasting ≥ 12 weeks + not explained by another diagnosis
· Multi-system, not limited to respiratory
Scale (Davis 2023 Nat Rev Microbiol)
· Among people with acute COVID who were not hospitalized, about 10-30% go on to Long COVID (an estimate)
· More women than men
· At least 65 million affected worldwide (Davis 2023 estimate)
The Davis 2023 review splits Long COVID from a vague "not fully recovered" into concrete clinical branches:
· CV (cardiovascular): palpitations / chest pain / tachycardia / myocarditis / POTS rise sharply
· NEURO (neurocognitive): "brain fog" + memory + attention + headache + migraine (Douaud 2022: brain scans before and after infection showed more grey-matter thinning in some smell-related regions and slightly more whole-brain shrinkage in people who had been infected — an observed association, mostly in mild cases)
· RESP (respiratory): dyspnea + chronic cough + reduced exercise tolerance
· PEM (fatigue / post-exertional malaise): heavy overlap with ME/CFS (the Davis 2023 review estimates that about half of people with Long COVID meet the diagnostic criteria for ME/CFS) — the key intersection between Long COVID and ME/CFS
· AUTO (autonomic): POTS / / temperature / sweating
· GI (gastrointestinal): diarrhea / bloating / new-onset
· MULTI (multi-system): joint pain / rash / menstrual disturbance / sexual function / smell-taste loss
Mechanistic hypotheses (coexisting, not singular)
· Viral persistence (antigen detected in tissue)
· Autoimmune activation (new autoantibodies)
· Microclots + vascular damage
· Dysbiosis + EBV / HHV-6 reactivation
· Vagus nerve + brainstem injury
Next: of these phenotypes, PEM is the most counter-intuitive — it is delayed, not "tire once, feel tired immediately".
