1 · Pain-free people are full of 'abnormal' scans
Even people with no back pain often show disc degeneration and bulges on MRI, more so with age, so these findings are mostly normal age-related changes.Scan someone with no back pain at all and you'll very likely still see a bulge or degeneration — and it gets more common with age.A review pooling large asymptomatic populations found: disc degeneration ~37% at age 20, ~96% by 80; disc bulge ~30% at 20, ~84% by 80. Every one of these people has no pain.
So an 'abnormality' on imaging is more like the spine's grey hairs and wrinkles — mostly a normal age-companion, not a verdict on your pain.
