Belly fat is not a quiet storehouse: the aromatase in fat converts testosterone to estrogen, which in turn dampens the brain's signal to the testes (Kelly 2015). In this loop, obesity lowering testosterone is a stronger effect than the reverse; obesity-related low testosterone is usually functional, and what needs treating is the obesity itself (Grossmann 2018).

Aromatase in belly fat converts testosterone into estradiol, and estradiol in turn turns down the signal the pituitary sends to the testes, so obesity often makes testosterone read low on a lab report; too little sleep also lowers the testosterone measured during the day. The same low number can come from obesity, from sleep or from a real endocrine problem, and whether to treat it is for a doctor to judge from symptoms and a repeat test.
