· FODMAP foods are completely harmless in healthy people, and most are healthy foods (vegetables / fruits / whole grains / legumes / dairy).
· Only in IBS patients (visceral hypersensitivity + microbiota dysbiosis) do they trigger symptoms.
· "FODMAP intolerance" ≠ food allergy: no IgE / immune mechanism.
The 5 categories + real food examples
① Oligosaccharides — fructans + galacto-oligosaccharides (GOS)
· Foods: wheat / rye / garlic / onion / leek / artichoke / legumes (soy / chickpeas / black beans) / cashews / pistachios.
· Mechanism: humans have no enzyme to digest fructans or GOS, so 100% reaches the colon for fermentation.
· Symptom contribution: the largest trigger in most IBS patients.
② Disaccharides — lactose
· Foods: milk / soft cheese / yogurt / ice cream / some milk powders.
· Mechanism: lactase insufficient in some populations (~90% East Asian / ~5% Northern European).
· Not unique to IBS: lactose intolerance is its own diagnosis.
③ Monosaccharides — free fructose excess
· Foods: honey / apple / pear / mango / watermelon / high-fructose corn syrup ().
· Mechanism: when fructose > glucose, GLUT5 saturates and excess fructose reaches the colon.
· Key: 1:1 glucose + fructose (e.g., sucrose / banana / berries) does not trigger — because glucose enables co-transport.
④ Polyols — sorbitol + mannitol + xylitol + maltitol
· Natural foods: apple / pear / peach / plum / mushrooms / cauliflower / watermelon.
· Processed foods: "sugar-free" gum / candy / diet foods (those labels are polyols).
· Mechanism: not because the molecules are large — sorbitol is 182 Da, smaller than the lactose (342 Da) in the category above. Polyols simply have no transporter: absorption is slow, passive paracellular diffusion only, so a large fraction reaches the colon → fermentation + osmotic water pull.
⑤ The "A" in "And" = "And" — this is an acronym trick, not a fifth category.
FODMAP additive effect
· Dose-dependent: one cup of milk + half an apple + one garlic clove + one slice of rye bread = cumulative FODMAP load.
· Individual thresholds vary: some tolerate 5 g/meal, others trigger at 1 g.
· Microbiota composition + transit time + visceral sensitivity: all variable.
